"marriage" - when? what?

Dear all,

over the last few months the topic of "marriage" has come up in a number of e-mail correspondences I have had with different people; there were two concerns mentioned in particular:
  1. elder couple (both man and woman are widowers) living together as "man and wife" without having gotten married by the state (or denomination), and
  2. Christian parents of a young man planning to get married wanting to do things right and not have the young folks commit fornication by having sex before being married.
These concerns bring up some rather basic questions: When is a couple married from a Biblical perspective / in God's sight? Is being married in God's sight dependent on a state issued marriage certificate? are there different "marriages" possible ... married in the State's eyes? married in a denomination's eyes? married in God's eyes?
How then should we as Christians consider these matters? how does an accurate understanding of the Biblical concept of "marriage" effect our dealing with people such as the elder couple mentioned above or the young folks with their concerns of sex before marriage?
